Know your benefits

Now more than ever, ongoing changes with health insurance plans make it especially important to understand your coverage and know the costs you must pay when visiting your physician, having tests, and undergoing procedures. For this reason, we recommend that you learn the specifics of your plan before upcoming health care visits.

Your insurance plan may require multiple copays, higher deductibles, and coinsurance. Coverage will depend on the type of plan you have chosen. You can learn the details of your policy by visiting your insurance company’s web site or contacting its customer/member services department. The back of your insurance card should include the phone numbers and web site addresses you’ll need.

If we can be of assistance, please contact your appointment office location to confirm participation.

If your plan is not included, please contact your insurance company at the number listed on the back of your insurance card for verification of participation. We also encourage you to ask about in-network providers for your best benefit.

Insurance Cards and Co-Payments / Deductibles

Please bring your insurance card to your appointment so that we can make a copy. If you are an existing patient, it is important that we have the most current copy of your insurance card.

Workers’ Compensation

If you are being seen under a Workers’ Compensation claim, it is important that we have your claim number, date of injury, employer at the time of injury, and all conditions. In addition, we will also need written authorization from your Managed Care Organization (MCO), or your employer, prior to your appointment. If you cannot provide this information by your appointment date, please call your appointment location office as soon as possible.
